Documentation Necessary for Kindergarten 

There are several items that are required by our State Law that need to be on file by the first day of school in order for SchoolCenter Picture your child to attend school.

1.  Early Childhood Screening:   If your child has not been screened, you may make an appointment for the free screening by calling Family Services, Mary Huberg at 645-1200.  The screenings take place at  Longfellow School and are approximately one hour long.

2.  Required immunizations:

    5 DTP (diphtheria, tetanus and pertussis)
    4 Polio
    3 Hepatitis B
    2 MMR (measles, mumps and rubella)
    2 Chicken Pox (varicella)

If you choose to file an exemption for immunizations, please complete an exemption form (also available from  your designated school's health office), have it notarized and return it to your child's school.  You may have your clinic fax your child's immunization record to your school.  Sibley's fax number is 507-645-3469.  Please mark the fax attention Jane Ehlers, RN.

3. A copy of your child's original birth certificate from the county in which the child was born. Hospital documents are not legal birth certificates therefore cannot be accepted.  A child must be five years old on or before September 1st in order to be eligible for kindergarten.

Please return any necessary documentation to the health office at Sibley Elementary as soon as possible, but no later than June 1, 2014.


**All documentation listed above must be on file by the first day of school in order for your child to attend kindergarten in accordance with the Minnesota State Law.
