If you are enrolling in the summer, leave a message for the appropriate counselor with your current contact information as well as dates and times when you will be available to meet.  Monday, August 29, 2011, is our designated new student registration day!  Please call in advance for an appointment.

 A parent / guardian must attend the enrollment meeting with their student.

 To expedite the registration process and to ensure that your student is placed in the appropriate classes, please provide the following items at the time of registration:
-Completed enrollment forms
-Student Information Sheet
-Personal History Form
-Home Language Questionnaire
-Request for Student Social Security Number
-Public Health Immunization Record and Legal Exemptions Form
-Health and Emergency Information
-Most recent report card
-Most recent transcript of all grades and credits (unofficial copies are acceptable)
-Most recent class schedule (if transferring during the school year)
-Course catalogues or other registration materials from previous school
-State-mandated testing results (GRAD exams)

 If applicable:

-Most current Individualized Education Plan
-Most current 504 Accommodation Plan
-Most current evaluation conducted by School Psychologist, Speech / Language Therapist, etc.
-Contact name and numbers for Special Education Case Manager
